Chronic Disease Prevention and Management
The Rhode Island Department of Health (RIDOH)’s Diabetes and Cardiovascular Health (DCH) Program provides information and resources to enable people to better prevent and manage chronic diseases, including type 2 diabetes and more.
Reducing diabetes and chronic disease risk can help strengthen your workforce. More than 1 in 3 Rhode Island adults have prediabetes and don't know it. Complications of diabetes and prediabetes (having higher blood glucose, or sugar, than is normal, but not yet as high as with diabetes) can affect the health and quality of life of your employees.
RIDOH’s DCH Program recommends the following tools and resources for employers:
Healm
Healm is a diabetes prevention Employer Learning Collaborative platform for the National Diabetes Prevention Program (National DPP), developed in collaboration with State Health Departments and the National Association of Chronic Disease Directors (NACDD). Healm offers tools to implement an effective diabetes prevention program benefit for employees.
NACDD Employer Coverage Landscape
The NACDD Employer Coverage Landscape shares private and public employers that offer coverage for the National Diabetes Prevention Program (National DPP) lifestyle change program as part of their benefit offerings and offers case studies and examples to draw from.
